Please use proper capitalization when filling out the form, i.e. John, not john or JOHN - San Jose, not san jose or SAN JOSE, etc. Thank you
PLEASE BE SURE you have not forgotten that you are already a member. We receive a duplicate application every once in a while, and that wastes our time as well as yours. All you need to do is check the membership list on the NAQCC web site at http://home.windstream.net/yoel/memberlist.html and search for your call there.
Fields with ** require an entry. If they are blank, you will be instructed to return here and fill in the required info.
If you experience other problems, please read the info below the form carefully.
Use TAB or your mouse to move between fields. Do not use ENTER.
You must have the following email addresses on your approved list or you may not hear from us. We DO NOT respond to those messages that say WE must add ourselves to your
list.



If after submitting the form you get any kind of error message, just send the required info in a regular email with the subject "NAQCC Membership Application" to the first of the three email addresses listed above the form.
If the form submitted correctly and you don't get an email issuing you a number within a week, be sure the three email addresses listed above the form are in your email approved list, then check back with us. We DO NOT respond to those who send us an email saying WE must add our email address to their approved list. That is YOUR responsibility, NOT OURS.
